Dose escalation for Wegovy follows a 16-week schedule: 0.25 mg (weeks 1–4), 0.5 mg (weeks 5–8), 1.0 mg (weeks 9–12), 1.7 mg (weeks 13–16), then 2.4 mg maintenance. This gradual increase minimizes GI side effects.
Administration
Semaglutide injections (Ozempic/Wegovy) come in pre-filled pens — no reconstitution required. Inject subcutaneously in the abdomen, thigh, or upper arm. Rotate injection sites. Administer on the same day each week at any time, with or without meals.
Rybelsus oral tablets must be taken on an empty stomach with no more than 4 oz of plain water, at least 30 minutes before food, drink, or other medications. This is critical for absorption.
Timing & Duration
Semaglutide is prescribed for long-term use — there is no cycling. Weight regain typically occurs after discontinuation (STEP 1 extension showed ~2/3 weight regain within 1 year). For type 2 diabetes, treatment is ongoing as long as benefit is maintained.
If a dose is missed, take it within 5 days of the scheduled day. If more than 5 days have passed, skip and resume the next scheduled dose.
Storage
- Unopened pens: Refrigerate at 2–8°C. Do not freeze.
- In-use pens: Store at room temperature (up to 30°C) or refrigerated for up to 56 days
- Rybelsus tablets: Store at room temperature in original blister packaging
