FDA-Approved Doses by Brand & Indication

Liraglutide is the same molecule across all approved brands; the dose target differs by indication. All doses are once-daily subcutaneous injection.

The starting dose of 0.6 mg/day is below the therapeutic range — it exists solely to acclimate the GI tract to GLP-1 receptor activation and reduce nausea. Doses below 1.2 mg do not produce meaningful glycemic or weight effects.[1]

Saxenda Weight-Management Escalation Schedule

If you cannot tolerate the increase to the next step (severe nausea, vomiting, dehydration), the FDA label recommends holding at the current step for an additional week before retrying. If 3.0 mg is not tolerated after extended titration, discontinuation should be considered — there is no FDA-approved maintenance dose below 3.0 mg for Saxenda.

If you've lost less than 4% of body weight after 16 weeks on 3.0 mg, the FDA label suggests reassessing treatment. Most non-responders are identified at this checkpoint.

Unlike Saxenda, Victoza does not require titration to the maximum dose. Many type-2 patients achieve target HbA1c at 1.2 mg/day. The dose is escalated to 1.8 mg only if 1.2 mg is insufficient for glycemic control.

Injection Technique

  • Pen format: liraglutide comes in pre-filled multi-dose pens. No reconstitution required.
  • Sites: abdomen (at least 5 cm from the navel), front of thigh, or back of upper arm. Rotate sites each day to prevent lipohypertrophy.
  • Timing: any time of day, with or without meals. Pick a time and stick to it for consistency.
  • Needle: use a fresh pen needle for each injection. Dispose of needles in an FDA-cleared sharps container.
  • Air bubble: always perform the flow check / priming dose before the first use of each new pen, per the device IFU.

Missed Dose Handling

From the FDA label:

  • Within 12 hours of usual time: take the dose as soon as you remember. Resume normal schedule the next day.
  • More than 12 hours late (close to the next scheduled dose): skip the missed dose, take the next scheduled dose. Do not double-dose.
  • More than 3 consecutive missed days: restart at 0.6 mg and re-titrate. GI tolerance can degrade rapidly off the drug, and resuming at the full dose causes severe nausea.

Drug-Interaction Timing (Gastric-Emptying Effects)

Liraglutide delays gastric emptying, which can alter absorption of oral medications. Practical implications:

  • Oral contraceptives: peak concentrations may be reduced. The FDA label notes Cmax reductions but the contraceptive effect is generally preserved. Consider backup contraception during initial titration.
  • Levothyroxine: separate by at least 30 minutes; monitor TSH after initiation.
  • Warfarin: monitor INR more frequently during dose changes.
  • Narrow-therapeutic-index drugs (digoxin, lithium, anticonvulsants): monitor levels.
  • Insulin / sulfonylureas: reduce dose to avoid hypoglycemia, especially during titration.

Storage

  • Unopened pens: refrigerate at 2–8°C until first use. Do not freeze.
  • In-use pens: can be stored at room temperature (up to 30°C / 86°F) or refrigerated for up to 30 days after first use. Discard after 30 days regardless.
  • Travel: insulated cooler with ice pack for trips longer than a few hours. Avoid direct contact with frozen gel packs.
  • Visual check: liraglutide should be clear and colorless. Discard if cloudy, discolored, or contains particulates.
