Common Side Effects

Cerebrolysin has a favorable safety profile with most side effects mild and transient:

In CASTA and CARS trials, adverse event rates in the Cerebrolysin group were comparable to placebo.

Serious & Rare Side Effects

  • Allergic reactions: Cerebrolysin is porcine-derived. Patients with hypersensitivity to pork products should not use it. Anaphylaxis is extremely rare
  • Seizures: Theoretical concern about seizure threshold lowering in patients with pre-existing epilepsy
  • Fever: Rarely reported; may indicate immune response to porcine peptides
  • Cardiovascular: No significant cardiovascular adverse events attributed to Cerebrolysin in clinical trials

Serious adverse event rates were not significantly different between Cerebrolysin and placebo in both CARS (n=208) and CASTA (n=1,070) trials.

Contraindications & Precautions

Cerebrolysin should not be used in:

  • Epilepsy: Particularly uncontrolled seizure disorders. Status epilepticus is a specific contraindication
  • Severe renal impairment
  • Allergic diathesis: History of severe reactions to porcine-derived products
  • Pregnancy and breastfeeding: Safety not established

Precautions:

  • Monitor for hypersensitivity during first administration
  • IV infusion should be slow (15–60 minutes)
  • Do not mix with solutions containing divalent cations (calcium, magnesium)

Safety vs. Alternative Nootropics

  • vs. Semax: Semax is intranasal with minimal side effects. Cerebrolysin requires injection with injection-site reactions and rare allergic risk
  • vs. Dihexa: Dihexa has very limited human safety data. Cerebrolysin has decades of clinical use across thousands of patients
  • vs. Selank: Selank is intranasal with minimal side effects and different mechanism (anxiolytic + immune modulation)
